Skilled nursing offers a wide range of services potentially deliverable outside of traditional institutional settings. Those needing skilled nursing services beyond that offered by home health increasingly desire to receive those services at home, including in senior housing settings. This session will cover the wide range of options hospital and skilled nursing providers are exploring to extend their services into the community, including hospital-at-home, skilled-nursing-at-home, and community-based special needs plans (SNPs).
